Fill up this form to provide consent for your dependants and yourself for the MHC Medical Benefits
To be eligible for the above benefit, Hirer (Main Hirer only) need to fulfil the following:
- Rent from Lion City Rentals and is currently (i.e. at the time of application) on an active contract.
- Must be on a 26 weeks / 52 weeks contract (new contracts or renewals) commencing from 10th January 2025 onwards.
- By submitting this form, you hereby declare that the information furnished above is true and accurate to the best of your knowledge. In the case where any of the above information is found to be false, untrue, misleading or misrepresenting of the facts, your application may be rejected.
- The hirer hereby allows Lion City Rentals and its affiliated partner(s) to use their personal information in accordance with the Data Protection Privacy Policy to facilitate the processing of medical credits.
Applications must fulfil the above requirements or incomplete applications will be rejected. Lion City Rentals will review all applications on a case-by-case basis.
- The medical credits are only applicable to the main hirer only. The benefit eligibility is tied to the contract duration and is valid only during the contract period stated, where the contract status is active.
- Upon any default of rental payment, the hirer will be disqualified from the scheme immediately.
- Lion City Rentals reserves the right to claw back any medical credits in case of breach of any terms and conditions above; the right to reject any application at its sole discretion; the right to make the final decision in any case of dispute and also the right to change or modify these terms and conditions without prior notice.
- Any early termination of your vehicle rental contract with Lion City Rentals may result in a clawback of benefit(s) awarded under this programme.
- Offered services are subject to other terms and conditions by MHC Asia.
- MHC Asia reserves the right to modify and make changes to any of these Terms and Conditions, including but not limited to the fees and charges relating to your use of the Websites/Mobile App and Online Facilities, without notice. All modifications to these Terms and Conditions will be reflected by the posting of the amended Terms and Conditions on the Websites/Mobile App. In addition, if there are any material changes to any of these Terms and Conditions, we will indicate on the Websites/Mobile App that our Terms and Conditions have changed.
- MHC Asia also reserves the right to suspend or terminate your account and refuse you any current or future use of any of our Websites/Mobile App or Online Facilities in our sole discretion, without any notice, for any reason whatsoever, including where we have reasons to believe that you have breached or acted inconsistently with the Terms and Conditions. We will not be liable in any way to you or to any third party for any termination of your access to the Websites/Mobile App and/or the Online Facilities.
